COVID-19 Office Policy Updates
Updated 8/9/2020
We are excited to open up our office to care for your visual needs. To provide a safe environment for our patients and staff, we are adhering to strict CDC cleaning protocols and following all recommended guidelines from our local officials.
Therefore, an approved mask (covering both nose and mouth) is required throughout the entire visit. Masks with vents are not allowed. Masks are available at our office for an additional fee.
Please continue checking our page for updates.
We are also limiting the number of people in our office by accepting appointments only (to help us enforce this policy, we ask that patients come alone or accompanied with a legal guardian if the patient is a minor). Please note that anyone with the following may be rescheduled:
- Been exposed to anyone with COVID in the last 30 days,
- Have experienced any symptoms such as fever, cough, loss of taste and/or smell in the last 30 days
When making an appointment, please have both vision and medical insurance info ready, if applicable.
New patients: please fill out and bring the appropriate forms to your appointment.
Please be patient with us as we work diligently to ensure a safe environment for everyone. Though things may be a bit different, our goal remains the same: providing quality eye exams to you and your loved ones, so please give us a call or an email today!
*** Please note that we do offer telemedicine services for certain types of visits for patients that are not comfortable with an in-person visit. Please email us at info@commonwealtheyecare.com to see if this is an option for you.
We appreciate your continued support and understanding during this unprecedented time. Stay safe!
